Fee Schedule 2016

Transition Rehab is a comprehensive, clinical care coordinating organization providing adults 21 and older who have a mental health disorder, dual diagnosis disorder and/or limited physical disability.

Transition Rehab offers

  • Certified Substance Abuse Evaluations;
  • Intensive Residential Rehabilitation living and treatment;
  • Clinical coordination;
  • Case Management;
  • Individual, Goup and Family Therapy;
  • Community Resource Coordination;

Initial Evaluation for Residential or Outpatient Clinical services - $150.00
(Includes mileage, individual meeting, family meeting & any medical providers)


Certified Substance Abuse Evaluations w/Drug Screen - $180.00 – In office only
(Required for courts, lawyers, parole, probation, Family Violence Prevention classes, Anger Management)


Intensive Residential Rehabilitative Services $1,500.00 per month (six month commitment)
(Clinical Case Management & Coordination, Housing, Individual & Group Therapy, Transportation Coordination, Medication Management, Life Skills Training, Life Skills Assessment, Peer Support, Team Building; Random Drug Screens (min 4); Wellness Planning, Transition Coordination; Vocational/Educational Skill Assessments and Community Support) – Gender Specific


Therapy Services (with yearly contract)
Individual Therapy - $450.00 per year for 15 sessions
Family Sessions - $600.00 per year for 15 sessions (maximum of 4)
Group Therapy - $300.00 per year for 20 group session


Respite Rehabilitation – minimum 3 days = $450.00; maximum 6.5 days $800.00
(Includes all Intensive Residential Rehabilitative Services and (1) 3 course meal per day) – Gender Specific


Adventure Respite Program – Quarterly Dates & cost posted on website; info emailed to listserve.

Family Respite Education Workshop – Monthly Dates and cost posted on website; info to listserve.


Coordinating and assessing all resources and medical provisions available, individually, to each client and family to assist with outcomes and moving forward toward sustainable independent living. If you currently have or need Medicaid, Medicare and/or private insurance, all eligible resources are also utilized to make sure each individuals full comprehensive service is wrapped and coordinated with collaborative providers who accept these benefits. Transition Rehab will provide you with a monthly invoice for contracted services not covered by your benefits, however, Transition Rehab is not responsible for filing insurance claims or for your insurance company's final decisions regarding reimbursement.
